Service Recipient Agreement
I understand Faith Impact is a volunteer-led ministry of Faith Lutheran Church, and is established to provide short-term assistance to those in need and/or those who find themselves in a “crisis” situation. In the event that volunteers/resources are not available, I understand Faith Impact, upon review of my application, may or may not be able to fulfill my current need. I acknowledge that all volunteers are assisting me not only to help me, but also to share the love of Jesus with me.
